A Kitchener man is dead and police are looking for a small black car after a crash west of Tavistock Tuesday night.

Police say around 10:50 p.m. Tuesday, a black compact car was travelling eastbound on Perth Line 26 towards Tavistock when it pulled out to pass a van.  An oncoming car steered onto the shoulder to avoid a collision.  The driver lost control in the gravel, veered across the roadway and was T-boned by the van.

The driver of the struck car was transported to a local hospital where he succumbed to his injuries.  Police have identified the man as 26-year-old Jason Ramtahal of Kitchener.

A Tavistock man was behind the wheel of the van.  He was transported to hospital where he was treated for minor injuries.

The black car, possibly a Kia or Honda, failed to stop or remain at the scene.

Perth County OPP are requesting that the driver of the car come forward. 

Perth Line 26 was closed through the night and re-opened around 7 a.m. Wednesday.
